Face Lift Surgery

  • As elsewhere, gravity and laxity of ourfacial tissues as we age leads to drooping of skin and deeper tissues in face and neck area. Face lift is a surgical procedure designed to remove the excess lax skin and tighten the underlying tissues to give a younger firm appearance to the face and neck. Facelift is the ultimate facial procedure which when done correctly has unparalleled effect in giving a younger, rejuvenated look that one can clearly see and appreciate 24 hours a day.

  • The face lift technique has evolved with time and you will come across a variety of procedures all grouped under the term “face lift”. However, the gold standard is a deep plane. Let’s talk about the common varieties of facelifts

    1. Comprehensive Face Lift – This is a full-face lift that works to lift the sagging mid face in its entirety. This is a major undertaking that addresses the skin as well as the underlying tissue. This is usually done by a “hidden” cut running from the front of your ear to behind it and then ending within the hair at the back. The most effective way to lift the facial tissue through this incision is when all three layers i.e. skin, tissue and facial muscles are lifted and repositioned in an elevated but natural position. This is the gold standard “deep plane face lift” and my preferred technique for everyone except for those with mild ageing of neck or midface. The deep plane face lift gives the best cosmetic outcome and the rejuvenation effect lasts the longest. The incisions are well hidden and are not apparent after a few weeks (please look at the pictures). Given the extent of surgery, you do need to schedule 2-3 weeks for recovery and will experience minor self-resolving inconvenience such as temporary numbness, bruising, swelling and you will have to take good care of the wound. The “investment” you put in during the recovery is truly worth it as there is no other procedure that can give you comparable outcome.

      However, “deep plane face lift” requires intense training and excellent surgical skills as it is a delicate precise surgery that requires the surgeon to be conversant with the anatomy of the face, particularly the tiny nerves that provide power to the face muscles i.e. Facial nerves. Therefore, unlike me, most surgeons do not offer deep plane face lift and perform a more superficial lift above the muscles in the name of comprehensive face lift. This has short lasting effect as the superficial surgery fails to address the sagging “foundation” of the face. This also leads to a tighter, unnatural feel as all the stretch is directed to the skin rather than the deeper layers.

    1. Weekend Face Lift / Thread face lift–As the name implies, this is a minimal face lift that elevates a limited part of face, usually the jowl area. The biggest attraction is the faster recovery (few days, hence the name “weekend”) because it is done through small incision usually just in under the ear with limited tissue manipulation. The usual way to do so now is by using a special designed absorbable thread “Silhouette Lift” the “One Thread Lift”. This works extremely well and is an excellent choice if you have very little ageing changes. The procedure itself is minimal enough to be done under local anaesthesia. But remember, the effects last just over a year and this technique will not be suitable if you have significant ageing changes.
